Where should a capped lead be placed?

Hi,
I recently had my PM replaced as well as a new lead installed. The old lead was capped and it is currently on top of the PM, OK... more like half on half off. It is not comfortable. I spoke to the doc and he said it must have moved, but it was there when I woke from the operation. Anyway, he is going to place it under the PM. I'm not sure that will be any better. So, what I would like to know is where is a capped lead normally placed?

My doc embeddedt the leads under PM and in muscle on my second. My third surgery : placer battery in mucsle

On top of the device carries the risk that they kind of "rub" their way through the skin, causing inflammation or even infection. Some doctors don't place the remaining of the leads under the device, but they burry it deep in the tissue. At the clinic where I'm being seen at, and where they perform about 1500 device implants/year, they put the leads and capped leads under the devices.
