Add lead

Doctor says they may have to go back in and add another lead to my pacemaker. I have two now, he said we may need three - anybody else had that problem? Also, he said that if they do that, they will "tuck" the pacemaker behind the chest muscle to prevent it from moving around like it has been. Wish me luck!

a 3 lead is called a CRT, or cardiac resynchronization therapy. If you put that in the search bar, you'll get many hits. A regular 2 lead make the heart beat fast enough and makes sure the ventricles beat when the atria does. The third lead forces the ventricles to beat in sync, so the left and right ventricles beat together. If your efficiency (EF, ejection fraction) has dropped, adding the third lead can help which can in turn give you more energy.

Placing the pm behind the muscle is unrelated to adding the new lead. It sounds like it's been bothering you but not enough to justify a new surgery? As long as they're going in anyway, it would make sense to reposition it if the current position isn't working for you. Mine is under the muscle and I love it. No visible lump, it never gets in my way.
