Jumping feeling in throat!!

I've had a pacemaker for 10 years now! 2 years ago my original wires broke down and I had 2 more added and a new System put onto my right side. These were active wires and only latest for 10 months before they failed. I had these removed and another active wire put into my ventricle, this perforated my heart so this was again removed and a passive wire put in. The Drs believe my heart didn't react well to the active wire. I haven't felt right though since my orginal wires, I'm still getting symptoms which all seem very positional, I seem to get a pause which jumps into my throat when I'm bending, turning my head or sometimes moving my arm, I also sometimes get a strong beating feeling near my ribs when lying down, sometimes breathing in will spark off symptoms :( I have had several checks which show the voltage on my ventricle wire is running at 0.4 which they are very happy with. They can't seem to catch these symtoms and when I try to explain they never know what I'm on about! My voltage is still set very high at 4 volts, I'm just wondering if having the voltage lowered may help? Im desperate to know if any of you guys have felt these symtoms and what it turned out to be? It's making me very nervous and getting me down :( just want to feel well again. You guys and your posts are so helpful to me :) any advice or suggestions would be great.

Forgot to mention I also feel my heart beating really strongly when I bend over, moving position will stop this.

To also add when I hold arm in a certain position I get a thumping racing heart sensation, does this sound like a voltage that's set too high or a lead position problem to you guys?
