Nothing is going to shake the leads loose. After the first 24-48 hours, scar tissue grows around them and holds them in place. After the first year, they are in there so good it takes a special laser to get them out.
One thing you may notice with a massager is your heart rate jumping up. If you pace with rate response, the pacer will be confused by the vibration into thinking you are working out so it will raise the rate. It's harmless but may be uncomfortable.
