Just had a checkup on the pacer and the Doc said the battery was running low (7 years).
I guess they change the pacer. Is this a big procedure?
a bit nervous I guess.

I have an ICD which is both a pacer/defib. I have no problems with mine and it has saved my life.

I had mine replaced last year. It's a very simple procedure. Less painful than the original insertion because they keep the leads in place, only change battery which is on top of the muscle. Since they don't have to cut into muscle it makes it less painful. I had my surgery on a Friday and was back at work Monday. Back in the gym and being active a week later.
