overdoing it

If you are exercising hard enough to burn 600 calories a day only 12 days out, you may simply be overdoing it.
re: swelling, some swelling and redness is normal. If you get heat, it is more swollen than a few days ago, you see red streaks or the incision oozes, you have a fever... those are all signs of infection so get it checked right away.
Have you had blood work to check your thyroid and hormones?
If you eat too few calories, your metabolism will slow down to compensate because your body thinks it's being starved so sometimes increasing the calories can actually help lose weight. This happened to me, I was eating 1200 calories a day, working out and burning 500-600 and not losing weight. On my chiropractor's advice, I increased it to a NET 1200, so if I work out 400, I eat 1600. I track on sparkpeople. And the weight is finally coming off even though I'm eating more, go figure. I have a lot more energy, too. Get some healthy fats in your diet, add in lean red meat for the iron, add some nutrient dense nuts, avocado, shrimp for the natural cholesterol. You don't want a high cholesterol diet, but our bodies need some. Same for fat, there are good and bad fats. Trans and saturated are bad but the fats in olive oil and nuts are good for us.
And I really believe whole eggs are better for us than egg whites, they have fat and calories but it's the good stuff that nourishes us, egg whites don't have much more than protein. Check this out

I'm a big fan of eggs, lots of good stuff for that 150 or so calories.

A couple of thoughts...

First at 12 days out you most likely still have some swelling / water retention for the surgery.
Your heart rate.. Are you exercising at 159 for the duration? We all have the aerobic sweet spot for fat buring (ie: getting the metabolism furnace going) for most this it between 60 and 70% of maximum HR. There are benefits of going higher, but how long can anyone sustain that? At 60 to 70% max, you are fairly comfortable and feeling good. (not huffing and puffing) You can do that level much longer.. also it causes your body to burn fat instead of protein, nitrogen, etc..
Others will chime in too.. hope that helps.
