I had an ICD implant 2 weeks ago today. I can't answer your question scientifically speaking but what i can say is that i've experience the same thing when eating, drinking etc. Its natural. I would be really suprised if the leads where that fragile. They would be doing lead replacments on 99% of people.

Is this something new, since your pacemaker implant? It might be that your diaphragm is being paced once in a while. If it becomes a real problem, talk to your EP and PM tech. There may be something that they can do. It's not going to do any damage but may indeed be annoying.

Periodic hickups are not dangerous...well unless you fart in sync with it then look out

I agree with the opinion about pacing the diaphram.  I've seen that happen with emergency pacer placements when I worked in CCU twenty something years ago.  It isn't dangerous, just very annoying.  Have it checked out if you've ruled everything else out.
