Pacemaker "Credit Card" ID Card

Good day
Can someone tell me where I can get a credit card type of pacemaker ID, I received the original ID piece of paper when I received my pacemaker in 2015 but it is getting a bit tatty and loosing the ink.

I see that you are in the UK. I telephoned the Medtronic head office in Watford ( tel no. 01923 212213 ) and received a credit-card type pacemaker card by return of post. They have records of every PM that they sell in the UK / name of recipient / hospital etc
I hope they are still happy to do this
By the way, the tatty paper version is the standardised European pacemaker ID card .... maybe Brexit will change things but i wouldn't hold your breath : -)

Just call or email or whatever method they offer, they will send another. In some cases you can call your doctors office and have them do it they deal with your vendor on a regular basis and probably have a more direct path into that company. either way aint no thing.

Get on Medtronic's website and request a new card. If you have trouble doing that, go to your EP's office and have his staff talk with the Medtronic rep and get one ordered that way.
You can also go to the hospital where you got your pacemaker and see if they can locate a Medtronic rep. They might also be able to help, or at least, point you in the right direction.
Medtronic should have sent you a permanent card but sometimes that information gets lost in the shuffle.
