Increase in pacing numbers

I am a 67 year old man and I have had my pacemaker installed on Jan., 2019 for a Mobitz type 2 block. I noticed that since that time by pacing has gone from AP 24.7%, VP 28.8% to my most recent AP 38.2% and VP 33.8%. Is this normal? I work out on my Peleton at least 3 times a week and  at times I push it quite a bit. I am wondering if it is safe to exercise at that level.

Unless your heart docs have told you not to push the exercise then you're fine to keep going, keep training, keep pushing.

Chances are that whatever process caused the HB (doesn't sound as though you have ischaemic heart disease) is progressive (probably fibrosis).  And may also affect your SA-node - the data show you've been needing atrial pacing since the start so it sounds as though you have more than just the HB.  So the numbers tell a story, but you feel good and want to keep going.

The benefits of vigorous exercise are considerable and well proven, and as long as your heart muscle can cope with this then keep going.

If you feel good, you are pacing the right amount. There is no number that is considered ideal, we are all different. It's common to need it more over time, these conditions tend to deteriorate  so the pacer picks up the slack automatically. As long as you feel good and are cleared for exercise, pushing it several times a week is the best thing you can do for your heart.

Pacing percentage often means absolutely nothing.  It can easily go from 0% to 100% just by changing the pacer's bottom rate. Even if that hasn't changed, it's not a number to be concerned with.  You have a pacemaker to make it right.

Exercise is something you should ask your doctors about but since they said nothing, I'd say it was safe to do anything you're able to do.  They prbably have your pacer set so you can't do anything stupid but it's probably a very conservative number.  If you run into the max rate, talk to your EP about raising it.  If there is a problem and raising it isn't wise, he'll certainly discuss it at that point. Otherwise, have at it.  That's why we have pacemakers, to make our lives as normal as possible.

Pacing numbers will fluctuate over time. Heart block tends to get worse not better so over time so the bottom pacing will most likely increase. Setting changes can also affect changes in pacing. They can pace you as much or as little as they want to help with symptoms.  If your active your lower rate limit may not have to be 60bpm. Mine is 50 and I"m very active. I only have heart block so the Brady pacing doesn't apply to me.  My doctor wanted my own heart to do as much work as it can on it's on and setting it at 60 would be pacing me unnecessarily.  The only goal with pacing is symptoms. As long as your feeling good. Then it's the right number for you.
